STANDARD: Non-verbatim, standard turnaround time of 48-72 hours. These are non-verbatim or “cleaned up” transcripts meaning that all ums, ahs, false starts, and non-important speech is omitted from the final transcript. My rate is $1.25 per audio minute.
VERBATIM: Transcripts requiring all ahs, ums, all utterances as well as sounds, including (Laugh), (Cough), (Telephone Ringing). My rate is $1.50 per audio minute.
MULTIPLE SPEAKERS: For more than (3) speakers, particularly of the same sex where differentiation is difficult (Focus Groups, Conference Calls, etc.), my rate is $1.50 per audio minute.
RUSH: For job requiring a turnaround of 24 Hours or LESS, my rate is $2.00 per audio minute.
DIFFICULT Additional charges may apply to difficult audio, including but not limited to, people not speaking directly on mic, foreign accents, or an extraordinary amount of background noise. These are reviewed and quoted on an individual basis. Standard charges for difficult audio would be an additional $10.00 per 30 minutes of Audio as these will take longer than the standard time to turnaround.
Typically, I am able to turnaround 60 minutes of audio within a 24-hour period and 90 minutes of audio within a 48-hour period with good quality audio.

By offering medical record summary services, we can help your office save time by organizing medical records for review and preparing an abstract or summary of the medical records highlighting important notes, procedures and complaints pertinent to a particular case saving countless billable hours for our office staff.
A medical records summary can help to assess the merits and demerits of a case. The summary service would help to identify and highlight the following: deviation or negligence from the standards of care, questionable happenings, particular areas requiring attention and pre-existing conditions that could have important bearing on the case. In the insurance industry, it gives direction for settlement and is useful for setting reserves.
A medical record summary is a concise version of the information found in a patient’s medical records. Arrangement and extensive research of the medical records such as progress notes, consultation notes, physician’s notes, and operating room records is done to prepare the summary. Though traditionally medical summaries are text-based, they may also include charts and tables. Information that may be included in this form of a summary includes: the cause of injury or accident, entry into and discharge from the hospital, immediate and ensuing care given, what diagnostic testing revealed, present diagnoses, medical expenditure and the patient’s deposition testimony.
We also provide summaries or abstracts of depositions and trials for your convenience.
Rate: $2.00 per page of finished product.
Medical records could be sent via FedEx or UPS or, if local to the Spring Hill, Florida, area (Tampa, St. Petersburg, Clearwater, etc.), I would be more than happy to provide pick up and delivery service of medical records to/from your offices. Mileage charge of $0.10 per mile, roundtrip, applies.

In the Spring Hill area? Cost is only $5 per visit, cash only, up to 10 documents to be notarized. Additional documents $1 each.
Need me to come to you? Mobile notary services available. I will be come to you at your location to notarize any documents you need notarized. Cost is $10 per visit, plus $0.10 per mile roundtrip for the convenience of having me come to you.
